. Approximately 140,000 deaths per year: cardio-neurovascular diseases remain the second leading cause of mortality in France (p. 7), while more than 70% of them are associated, globally, with avoidable factors (p. 3). This document sets the national prevention framework until 2030: quantified targets (p. 12) and 36 actions that mobilise particularly primary care, coordinated practice, occupational health and schools (p. 15-24). It documents social, territorial inequalities and between women and men (p. 6-10), situates it in relation to the law of 27 July 2026 and the plans on determinants (p. 11) and proposes 15 benchmarks for users and professionals (p. 25-26).

1️⃣ Summary

Cardio-neurovascular diseases are the second leading cause of mortality in France, with approximately 140,000 deaths per year (p. 7). This roadmap covers the promotion of cardiovascular health and the prevention and management of risk (prevention, detection, management of modifiable factors) at the level of individuals and at-risk groups; it does not cover specialized care pathways (p. 11). It claims to be complementary to interministerial plans on determinants, which it does not detail (p. 11, 15). It targets the population and identifies at-risk groups: middle-aged adults, women, economic or territorial vulnerability, disability status, elderly people (p. 10). It sets target indicators for 2030 (p. 12), 5 flagship actions (p. 13) and 4 axes: promotion to the population and stakeholders; mobilization of primary care for detection; resources for comprehensive management, particularly for high-risk patients; knowledge, monitoring and evaluation (p. 15-24). It concludes with 15 benchmarks (p. 25-26).

2️⃣ What to remember

1️⃣ Target indicators for 2030 (status: objectives). Reduction of standardized rates of cardio-neurovascular mortality compared to 2023: −15% for overall mortality, −10% for premature mortality (< 75 years); +10 percentage points of the population at high CV score, −5 points at degraded score; 90% of the population aware of the symptoms and calling for emergency services, 75% trained in first aid (p. 12, source indicated: Public Health France).

2️⃣ Detection entrusted to primary care (status: actions to be taken). The axis II notably plans to promote My Prevention Assessment as a tool for cardiovascular prevention (action 2.1), to support the detection of hypertension by empowering community pharmacists and other providers in accordance with the law of 27 July 2026 (2.5) and to mobilise coordinated practice structures (2.8) (p. 17).

3️⃣ Findings of inequalities and measures for targeted audiences. Findings : incidence of hospitalisations 30% higher in disadvantaged communities, degraded score among 26% of 18-74 year-olds without a baccalaureate compared to 9% beyond, higher mortality in the DROM (p. 10). Measures dedicated to women (measure 3), to fragile elderly people (measure 4) and to vulnerable or hard-to-reach individuals (measure 5) (p. 18).

3️⃣ To act

1️⃣ Rely on the 15 benchmarks for cardiovascular health (p. 25-26). Existing tool cited by the document. It presents them as benchmarks for users and professionals: tobacco, diet, physical activity, alcohol, stress, sleep, blood pressure, warning signs of heart attack and stroke, family history, treatments.

2️⃣ My Prevention Assessment as a tool for cardiovascular prevention (p. 13, 17). Action explicitly provided for by the document (flagship action 2, action 2.1). The document plans to promote this system; it does not specify who will carry out this promotion, nor its methods, nor its timetable.

3️⃣ Life-saving actions: communication "cardiac arrest, heart attack, stroke: I act" (p. 22). Preparation for an action announced by the document (action 9.3). The document provides communication tools available on santé.fr and an encouragement to train in life-saving actions; it does not describe these tools or their availability date.

4️⃣ 3 practical questions

1️⃣ Which groups are identified as at risk? Mid-life adults, women, people in economic or territorial vulnerability, those with disabilities, elderly people, in a logic of proportionate universality (p. 10).

2️⃣ Does the roadmap cover specialised care? No : it does not cover the specialised care pathways for cardio-neurovascular diseases or related conditions (p. 11), and the action plans of the interministerial plans on the determinants are not specified (p. 15).

3️⃣ What warning signs should be made known? Heart attack : chest pain or pressure, sudden shortness of breath, discomfort. Stroke: sudden weakness on one side of the body, speech disturbance, loss of vision, severe headache, even transitory. Never trivialise: call 15 (p. 26).
